Johto existed when Kanto did and in all the games it would have existed, just no accessible and not in the game.
Gold/Silver/Crystal are 4 years ahead of the other games and the story-line that is in Johto is after Pearl/Diamond are already over.
That's what I meant. Not that Johto just spontaneously appeared 4 years after Diamond/Pearl. In fact, there are a few references made to Johto in Diamond/Pearl.
